Viewing this website, submitting a request form, sending an email, or otherwise communicating with us through this website does not create a physician-patient relationship between you and the practice or any of its clinicians.

A physician-patient relationship is established only when the practice has accepted you as a patient and an appointment has been arranged. Until that time we are unable to provide clinical advice, issue prescriptions, or assume responsibility for your care.

Emergencies

This website is not monitored continuously and must not be used to seek urgent or emergency assistance. A request submitted here may not be seen for some time.

Appointment Requests

Request forms on this website are a means of asking the practice to contact you. Submitting one does not book, confirm, or guarantee an appointment, and does not guarantee that the practice is accepting new patients or participates in your insurance plan.

You agree to provide accurate information when submitting a request, and not to submit information about another person without their authority. Please do not include medical details in these forms.
