The plan has stalled
Several trials in, with partial or no response, and the next step being offered is another variation on the same theme.

We’ll call you back.
We use this only to call you back. Please leave out medical details — this is not a secure clinical channel. Or call 914-877-1200.
Asking for a second opinion is not disloyalty to your clinician. In complex cases it is standard practice.

Some cases are genuinely hard. Symptoms that span two diagnoses, four medication trials with partial responses, a picture that shifted after a medical event, a diagnosis given years ago that nobody has revisited since. In those situations a fresh, structured read is worth more than another dose adjustment.
A consultation here is a defined piece of work rather than an open-ended transfer of care. We review the records you provide, take our own history, and set out where we agree with the current formulation, where we do not, and what we would want to know next.
You receive that in writing, in language you and your treating clinician can both use. Most patients take it back to the psychiatrist they already have. Some choose to continue here. Both outcomes are fine — the point is that the decision is now an informed one.
Several trials in, with partial or no response, and the next step being offered is another variation on the same theme.
Something about the current formulation has never sat right with you, or the picture has changed since it was made.
You are weighing something consequential — a new class of medication, neuromodulation, a change in level of care — and want an independent read first.
You send what you have: prior evaluations, medication history with doses and durations, testing, discharge summaries, relevant medical work-up.
We take the history fresh rather than inheriting the previous framing, so the review is genuinely independent.
Where we agree, where we differ, what we would do next and why, and what evidence would change that recommendation.
The opinion is written to be usable by your existing clinician. If you would rather continue care here, we can do that instead.
What patients ask about second opinions & consultation.
If yours is not here, ask it directly — we would rather answer before an appointment than after one.
Ask a questionNo. Most second opinions are exactly that — a one-off consultation, with the findings returned to you and, if you wish, to your treating clinician. Continuing here is an option, never an expectation.
